Hernia Clinic

Anybody can get a hernia – Hernia affects men and women, boys and girls, and many babies are born with them. In adults the only treatment for a hernia is surgical and most physicians believe people avoid treating their hernias because they fear painful surgery. Today, there is little reason to fear. Hernia surgery is usually performed on an outpatient basis and patients are able to return to most normal activities in a matter of a few days.

What is a hernia?

A hernia occurs when the inside layers of the abdominal wall weaken then bulge or tear. The inner lining of the abdomen pushes through the weakened area to form a balloon-like sac. This, in turn, can cause a loop of intestine or abdominal tissue to slip into the sac, causing pain and other potentially serious health problems.

Men and women of all ages can have hernias. Hernias usually occur either because of a natural weakness in the abdominal wall or from excessive strain on the abdominal wall, such as the strain from heavy lifting, substantial weight gain, persistent coughing, or difficulty with bowel movements or urination.

Types of hernia

There are four primary types of hernias:

Incisional or ventral: a hernia that appears in the abdomen on the site of a previous surgery is known as ventral or incisional hernia.

This can happen soon after the operation or many years later and can vary in size from small to large.

Inguinal: an inguinal hernia is quite obvious and appears as a bulge in the inguinal (groin) area. the bulge can easily be seen and felt once the patient stands up and on lying down it will be disappeared. One can have inguinal hernia in one or both the sides.

Femoral: these hernias along with inguinal hernias are groin hernias. they are much more common in women but can occur in men too.

Umbilical: occurs around the navel and is usually present at birth, though it may not become a problem until adulthood.

What are the symptoms of hernias?

  • The most common symptoms is the lump or noticeable protrusion in the groin area or in the abdomen
  • Sometimes it is painful.
  • A vague feeling of fullness

A small hernia may not be noticed and be found during routine examination. the lump often disappears when the patient is lying down. however a hernia can be dangerous if it’s get trapped at the exit. there would be pain and the persist even after lying down. this is emergency situation and the patient needs an urgent visit to a hospital, as urgent surgery is needed.

How can a hernia be repaired?

Hernias usually need to be surgically repaired to prevent intestinal damage and further complications. The surgery takes about an hour and is usually performed on an outpatient basis (which means the patient can go home the same day of the procedure).

This surgery often involves placement of a mess and may be performed by an open repair (small incision over the herniated area) or by laparoscopic surgery (minimally invasive – keyhole surgery). Your surgeon will determine the best method of repair for your individual situation.

In the laparoscopic method usually 3 small cuts are made of 0.5 to 1 cm in length. Special instrument are used and operation is done viewing the field on a TV monitor.

Advantages of key hole hernia surgery

  • The small incisions results in less pain and earlier return to work
  • The positioning of the mess (inside the defect area) is mechanically better than placing it outside as in open surgery
  • The magnified view during laparoscopic surgery allows meticulous dissection and prevention of any injury to adjacent structure.
  • For recurrent hernias Keyhole surgery is again useful. a surgeon using open technique has to dissect through tissues that is already very scarred due to the first operation. This can lead to increased chances of complication. if the operation is done with the Key hole technique, the Hernia is approached from inside where the tissues have not been affected during first operation. This makes easier for the surgeon and less painful for the patient.

After keyhole surgery

By the time patient wakes up from the anesthesia the repaired hernia is already well patched up. naturally the patient will be a little tender in the first few days but after a week or two there is no need to limit his/her activities. However be prudent and use caution regarding activities undertaken and before doing any strenuous activity do take advice of your doctor.

Can hernia come back?

A hernia repaired by an experienced surgeon is the key factor for low recurrence rate. The recurrence rate of inguinal hernia in best hospitals (open or laparoscopic) is less than 1%.
